Ophthalmology Associates, P.S.C. is open Monday through Friday from 8:30 – 5:00. Please call (502) 897-9881 or toll free (800) 688-9881 to schedule a date and time that is convenient for you. Insurance
We participate with most major insurance plans. It is your responsibility to contact your insurance carrier to determine your benefits and requirements on referrals. If we do participate with your particular insurance carrier, and your plan covers the service we will provide, we will be pleased to file your claim. Any deductible or co-pay amount will be due at time of service.
If we do not participate with your insurance carrier, you may want to ask your carrier if you have out of network benefits. We will be happy to file your claim if you have out of network benefits. Any deductible or portion due from the patient will be due at the time of service.
Medicare
Ophthalmology Associates, P.S.C. participates with the Federal Medicare Program and most of the Medicare Replacement Plans. Medicare covers exams that are related to eye diseases or conditions such as glaucoma, diabetes, macular degeneration, detached and torn retina, and cataracts to mention a few.
Payment Policy
Co-payments and deductibles, if they have not been met, are due at time of service. You are also responsible for non-covered services that your insurance plan does not cover. An example of a non-covered service is our $25.00 refraction fee. The refraction is a diagnostic measure that is performed to determine the amount of correction you need to see well. A refraction is done in both medical and non-medical cases.
